What is Palliative Care?
Palliative Care is a relatively new specialty offered to individuals with serious and chronic illnesses such as congestive heart failure, cancer, chronic obstructive pulmonary disease (COPD), AIDS, kidney failure and Alzheimer’s.
Palliative Care does not indicate that there is no hope for recovery. In fact, it can be provided while patients pursue a cure. Some patients do recover and move permanently out of Palliative Care while others with chronic diseases, such as COPD, may move in and out of Palliative Care as necessary.
The Palliative Care Team will work side by side with a patient’s Primary Care Physician, or Specialty Physician, to appropriately manage a patient’s pain and symptoms, while providing social support and resources to the patient’s family and friends.
Palliative Care will help you improve your ability to go through medical treatments, while you continue on with your daily life, and help you match your goals to your treatment choices.

Hospice Care
Hospice is a service that is provided to those who have a life limiting diagnosis.
The care is provided in a patient’s home environment. Home can be a private residence, group home, assisted living, long term care facility, or a nursing home. The service allows for the Hospice Interdisciplinary Team to come in and help provide care.

Private Duty Care is a wonderful service provided to individuals and their families when additional help is needed within the home.
This service provides a caregiver to the patient and/or their family who can assist with a wide variety of things that can often be challenging.

Home Health
Home Health is a service that focuses on helping people rehabilitate after an illness or surgery.
The services offered by Home Health agencies are: Skilled Nursing, Physical Therapy, Occupational Therapy, and Speech Therapy. This service is for individuals that are home bound or have difficulty getting out of the house for medical treatment.
